CMLSO Requirements

Ready to apply?

In order for an individual to qualify to take the CMLSO examination, the following requirements must be met:

  1. Candidates must have a 4 year degree from an accredited institution or be a licensed medical professional or they may either have a 2 year AS degree or high school diploma and advanced education and training in their individual area of practice.
  2. One (1) year of experience with laser safety or acting as LSO, having performed the specific responsibilities outlined in the ANSI Z136.3 Safe Use of Lasers in Health Care Facilities standard.
  3. Two (2) professional reference statements or letters of recommendation from the applicant's supervisor and/or other knowledgeable person(s) stating that the applicant has performed the specific responsibilities of an LSO. (Click here to get Reference Questionnaire)
  4. A certificate of completion of a BLS approved Laser Safety Officer course. (In special cases, this requirement may be waived by the BLS upon application with supporting documentation.)
