Exam Eligibility Requirements

Only eligible applicants may be approved to take an exam. Each exam has unique requirements:

CMLSO Exam Eligibility Requirements

Experience

One (1) Year Experience performing the responsibilities of an LSO as outlined in ANSI Z136.1

One (1) Year Experience performing the responsibilities of an MLSO as outlined in ANSI Z136.3

Education

  • 4-year degree from an accredited institution

OR

  • 2-year AS degree

OR

  • High school diploma and substantial experience in laser safety
  • 4-year degree from an accredited institution

OR

  • Be a licensed medical professional

OR

  • 2-year AS degree or high school diploma and substantial experience in laser safety.

Training

A certificate of completion from a BLS approved LSO training course

A certificate of completion from a BLS approved MLSO training course

References

Two (2) professional reference questionnaires or letters of recommendation from the applicant’s supervisor and/or other knowledgeable person(s) stating that the applicant has performed specific responsibilities of the LSO. These should be emailed directly from the reference to BLS.

Several organizations offer LSO or MLSO training courses that satisfy the eligibility requirement for either the CLSO or the CMLSO exam. Window of Approval

Upon approval to sit for an exam, a candidate is given a 2-year window of approval in which to take and pass the exam. The candidate may schedule a maximum of two exam attempts during this window (retake fees apply). If the candidate does not take/pass the exam during that window, the candidate must re-apply to take the exam with new application materials and new fees in order to open a new 2-year window, only after the first 2-year window has closed.
